What are the characteristics of melasma compared to other blemishes?

Melasma has slightly different characteristics than simply excessive melanin production leading to skin pigmentation.

It begins with the overactivation of pigment cells called melanocytes located in the deep layers of the skin.

The melanin produced by these cells spreads across the entire skin in a wide and repetitive pattern.

Ultimately, melanin penetrates into the dermis, taking on the form of a complex, long-lasting pigmentation disorder.

External stimuli such as UV exposure and hormonal changes are particular factors that further activate melanocytes.

It is known that subtle inflammatory reactions and specific vascular factors within the skin also play a role in this process.

Therefore, melasma is characterized by showing a diffuse pigmentation pattern with ill-defined borders, rather than a simple spot.

Histologically, the number of melanocytes is generally within the normal range.

However, the size of the cells and the dendrites, which are dendritic structures that transfer melanin, show a distinct increase.

This leads to a more extensive transfer of melanin to surrounding cells.

Occasionally, some patients may experience fine capillary dilation or flushing reactions in the melasma area.

When such patterns appear, it is sometimes categorized and explained as 'vascular melasma'.

Rather than viewing melasma as a single disease in itself, it is more accurate to describe it as the skin reacting sensitively to certain stimuli,

and as a result of various stimuli acting on a structurally unstable skin condition.

Therefore, rather than simply removing pigment, treatment that considers both the causes of melasma and underlying structural issues within the skin is important.
